Add troubleshooting section to docs #75

Summary
Add an FAQ or troubleshooting section to the docs.
In particular, dropping records is often because the person ids aren't matching between input files and the 'person file' (also, this is confusing because there's a person file in the output, which is different to the person file in the cmd line args).
There's a couple of reasons this can occur, but a particularly nasty one is where there is a leading '0' in the person id). see https://github.com/Health-Informatics-UoN/carrot-mapper/discussions/960 for full explanation.
