Plus other assorted fixes.

If no address has been set for the daemon in the configuration, read
the environment variables _AWS_XRAY_DAEMON_ADDRESS and
_AWS_XRAY_DAEMON_PORT.  This should hopefully make this work under
Lambda.
Don't log duplicate messages about not reaching instance meta
endpoint.  No need for access keys when accessing meta endpoint.
