/audit-docs PRD handling, per the owner's 2026-10-04 review of the trial run:

  • A PRD whose feature is mid-build is skipped, like an in-flight TRD.
  • A PRD requirement rewritten because the product behaves differently is listed separately in the change set for confirmation, and gets a dated row in the PRD's own changelog (a missing row is reported).
  • A non-goal the code contradicts is left as written and reported.

Stacked on #26 (feature/docs-as-built/trd): merge #26 first; this PR's diff then shrinks to its own commits.

Audit: proceed with caveats — no PRD (decided in session, checked against the TRD only); behaviour-vs-mechanism judgement not yet tested live; the one test gap found was fixed. Report: https://claude.ai/artifact/4jyABKMkMT69MhvHDMZs91

Summary by CodeRabbit

  • New Features

    • Added /audit-docs to review PRDs, TRDs, and other documentation against the built code. Choose a comprehensive review or, by default, focus on eligible PRDs and TRDs changed since the last run.
    • Reviews run on a local branch and can produce document updates, indexes, and a change-set report before returning to your original branch. The command does not push or open a pull request.
    • Reports highlight behavior changes, contradicted non-goals, and missing changelog entries for owner review.
  • Documentation

    • Added command guidance and review criteria, including configurable review thresholds and comprehensive-mode behavior.

Merge Risk: 🟡 Moderate · up to eb569

After a failed run, /audit-docs can silently skip documents it promised to recheck. Light runs can also review and edit documents outside the commits since the last run. Fix both, and make the apply step stop when git fails, before merging.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to eb569

The new audit process includes useful safeguards, but its commits can include staged changes those safeguards never approved. Delivery remains local until the owner publishes it, limiting exposure. Execution permissions and interruption recovery also remain partly uncertain.

Retained concerns

  • Medium · security · observed: The commit boundary does not enforce the gate's approved change set. commitBatch stages approved edited paths but then commits everything already staged; finalize also commits the existing index before cleaning strays. Staged hook-state changes are exempt from preparation and cleanup. Additionally, an in-batch staged deletion accompanied by outcome edited survives cleanup, is skipped because the file is missing, and never undergoes the removal reference check. These paths allow unapproved changes or removals into the review branch. Remote publication still requires a separate owner action.
Security review details

Security Blast Radius

  • inferred — The demonstrated commit-boundary weakness affects the local review branch and shared repository index, not just the approved documentation paths. Its staged-change exposure includes exempt tracked state. The larger host, credential, or network scope depends on production execution permissions that were not established; remote exposure requires subsequent publication.

Security Findings and Attack Paths

  • inferred — A reviewer capable of staging a deletion can report edited for an in-batch document. Apply treats it as claimed, skips the missing file, and never submits it to removal checking; delivery commits the staged deletion anyway. Repository-content manipulation could induce this behavior, but no successful prompt-injection exploit or production permission inheritance was verified. Independently, already-staged exempt state can enter a commit without reviewer misconduct.

Trust Boundaries and Controls

  • observed — The workflow tells reviewers to edit only their assigned document and never perform Git writes. Its writer invocations pass a response schema but no explicit tool or filesystem restriction. Existing repository configuration allows Git and general execution commands; that configuration is unchanged by this PR. The new caller's effective runtime confinement remains unresolved, so instruction text and response schemas are not treated as proof of capability isolation.

Resilience and Maintainability Implications

  • observed — Recovery retains diagnostic files but does not establish an atomic tree-and-result transition. The hook-state exclusion test leaves its state modification unstaged, so it does not demonstrate isolation of the index during failure, repetition, or an unexpected staged change.

Hardening Proposals

  • proposed — Before every commit, verify the complete staged path-and-operation set against approved edits, approved removals, or the explicit finalization outputs. Reject unexpected deletions and staged state rather than relying on selective git add.
  • proposed — Use enforceable per-reviewer filesystem and tool restrictions, keep validation code and run metadata outside reviewer write authority, and define recovery that verifies cleanup before resuming delivery. These are hardening proposals, not verified production controls.

Correct the FS-18 criterion before certifying it.

FS-18 requires the confirmation list in STATE. The PR objective places that list in ISSUES. The first-round claim also reports ISSUES, but the final reason says STATE and marks FS-18 met. Align the criterion with the intended readout, then correct the final verification records and evidence claim. As per the PR objectives, the confirmation list belongs under ISSUES.

Put the confirmation list in ISSUES in every verification record. The reviewed command puts behaviourChanges in ISSUES, but these three records say STATE. This makes the FS-18 criterion and its reported result inaccurate.

  • .trd-state/audit-docs-prd-handling/success-definition.md#L35-L35: change FS-18 to require the confirmation list in ISSUES.
  • .trd-state/audit-docs-prd-handling/verification-report.md#L32-L32: correct the met row and check its cited evidence against the corrected criterion.

Restrict light-run dispatch to the commit window.

In light mode, reviewable includes every tracked PRD and TRD whenever the window contains any commit. The workflow then scores each document, and a high score can route an out-of-window document to review. Filter light-run candidates so documents outside marker..HEAD cannot enter a review batch, then reassess FS-10.

Separate text and harness proof from live-run proof.

The success definition reserves live-run portions of FS-13, FS-25, FS-27, and FS-29, but this report counts each full criterion as proven while declaring no full-environment run. State which portions the artifacts prove. Mark the remaining portions unverified unless separate live evidence exists. The current “28 of 30 proven” coverage claim exceeds the stated evidence.

Persist failed batches as retry work.

When a workflow batch fails, both command copies say its documents will be reviewed on the next run. Finalization still advances the marker. A later light run can score those documents below MEDIUM and skip review; an empty commit window creates no batch at all.

Persist failed document paths and force pending paths through review regardless of the light-run score. Clear them only after a successful review. Apply this retry flow to both command copies.

Check git's exit status in the apply gate. Do not fail open.

git() in docs-audit-apply.js never throws on a non-zero exit. Two failures follow:

  • If git diff HEAD fails, addedLines returns []. The doc then passes the D12 signpost check and the changelog check.
  • If a checkout HEAD call fails, markReverted still reports the doc as reverted.

referenceCheck already treats a failed git grep as "unknown". Apply the same rule to the diff and to the revert calls.

  • packages/core/lib/docs-audit-apply.js#L93-L99: throw when git diff exits non-zero. Use a throwing helper for every checkout and reset call in apply.
  • .claude/lib/docs-audit-apply.js#L93-L99: copy the same change byte-for-byte into the mirror.

Based on learnings: "Let git errors for actionable, user-fixable conditions … propagate/surface."

Add set -euo pipefail as the repository guideline requires.

The file does not enable set -euo pipefail. Add it after the helper source line. Then run the fixture tests once to confirm that the helpers in helpers/setup.sh tolerate -u. Keep the existing ${VAR:-} defaults.

Select the change set by the branch's run id, not by head -1.

The test merges run 1's branch into main before run 3. The run-3 branch is cut from that main, so its .trd-state/_docs-audit/runs/ holds two change sets: run 1's and run 3's.

head -1 returns the first name in alphabetical order. Both names are <date>-<sha7>.md with the same date, so the order depends on the random SHA prefix. The "run 3 … Mode: light" test can therefore read run 1's comprehensive change set and fail even when the code is correct.

finalize names the branch docs-audit/<runId> and the file <runId>.md. Build the path from the branch name.
