Remove duplicates and fix typos

Typo fixes and formatting improvements to make common linters happy

Add backticks around words with angle brackets so that they won't be treated as HTML tags

Remove duplicate questions:

• 01 & 45. Removed 45
• 21 & 39. Removed 39
• 24 & 43. Removed 43
• 41 & 69. Removed 41 (The four choices for question 41 had been mistakenly copied from question 40. Question 69 has the correct choices and is therefore kept.)

Remove incorrect question(s) that cannot be fixed due to lack of more information:

• Q20. Not enough choices given to make the question meaningful

Fix numbering as a result of previous deletions

Add one new question at the end
