issue: /review spawns new threads on project level, or even on different projects

When I use the /review command, Codex creates new agents to perform the review.

I think those should be created as sub-agents but instead they are created on project level. One time it even created an agent on a different project.

What should the normal behavior be? I'll try to tackle it later in the day.
