Hito 0: cost foundation (pricing snapshot + single-call cost) - #1

Add the deterministic cost foundation Augur builds on: - cost/: pure single-call cost computation from a pricing snapshot. Usage models CachedTokens as a subset of InputTokens (matching OpenAI/Anthropic usage reporting); Validate rejects corrupt traces rather than under-billing. Unknown models are a hard error. - cost/pricing.go: YAML loader with an on-disk type decoupled from the computation type; rejects non per_mtok units and empty snapshots; omitted cached_input falls back to the input rate. - pricing.yaml: dated snapshot (2026-06-21), OpenAI + Anthropic. - SPEC.md: project spec and milestone plan. - Exhaustive unit tests (cache split, fully-cached, sub-cent, invalid usage, unknown model) plus a guard that the committed pricing.yaml parses and prices.
