docs: close issue 84 coverage-truth documentation

Purpose

Close the remaining canonical-documentation portion of issue #84 after #71 integrated the Test Strategy, gap audit, and traceability graph.

Verified defect

Fresh protected-main inspection found one surviving contradiction in CLAUDE.md: contributor guidance still said /* v8 ignore */ markers in src/index.ts were deliberate even though issue #84 source repair removed the broad credential/security exclusions and the newly protected docs/TEST_STRATEGY.md makes their reintroduction a regression.

Test-first repair

• RED contract 3df14a9378a748e8e58fe4c4d9fedf05bfc0b063 adds a documentation test that rejects the stale statement and requires contributor guidance to link to docs/TEST_STRATEGY.md and state the surviving broad-exclusion invariant. Its first PR-triggered runs were cancelled when the branch immediately advanced; cancellation is not counted as passing or as CI-proven RED.
• GREEN candidate c0006b50066f6f3ba883138a95cfbb476cecc86e replaces only the stale CLAUDE coverage sentence: broad credential/security V8 exclusions are regressions and the canonical exact-coverage policy lives in docs/TEST_STRATEGY.md.

CHANGELOG.md was inspected separately and did not contain the stale v8 ignore claim, so no churn was added there merely to satisfy the issue checkbox.
