chore(openspec): every change and spec names its screens

Decision 150: every open OpenSpec change and every main spec names the board (screen design) it belongs to, so build lanes never look it up.
Adds one new file per directory, screens.md, under openspec/changes/* (not archive) and openspec/specs/*. No proposal.md, design.md, tasks.md or spec.md is touched. Boards are collected from specScreens, matrix rows (built.spec / built.change and their screen), and the design-system cap notes in screens-src/zuiddrecht/apps/learniq.json. Board URL shape: https://identity.conduction.nl/screens/board?id=learniq/<Board>.

Counts (141 directories): 79 with boards, 35 with a no-screen reason, 27 on the design backlog (decision 157, 22 proposed boards in the build-all design-backlog.md), 0 with no board found.

Generator: ~/memcap-work/build-all/bin/spec-screens.py. Docs-only change; nothing else verified.
