Summary

Phase 94 open issue triage patch release.

Move 5 Fixed entries that were added to [4.16.0] after the v4.16.0 release (PR #220 on 2026-06-18) into a new [4.16.1] - 2026-06-19 section, and bump version to 4.16.1.

Issues addressed

• harness-review and harness-release do not persist the review verdict to review-result.json, so an APPROVE produced outside harness-work cannot pass the commit guard #218 — harness-review / harness-release APPROVE was not persisted to .claude/state/review-result.json
• PostToolUse cleanup deletes the approval on every commit, so in harness-release's multi-commit flow (work + bump) the later commit is blocked by the commit guard #219 — release bump commit blocked by approval consumption (bookkeeping-only commits now exempt)
• Skill listing budget overflow: 28 skills sent to LLM = 9,089 chars (6,000 budget) #200 — Skill listing budget overflow short-term mitigation (top 10 verbose descriptions trimmed)
• English version #173 — Language / i18n setup documentation reachable from README (docs/i18n.md)
• Trigger ( This request triggered cyber-related safeguards ) #172 — Reviewer cyber-related safeguard mitigation (narrow security finding instruction + docs/known-limitations.md)

Version bump

• VERSION: 4.16.0 -> 4.16.1
• All version surfaces synced (plugin.json, codex/cursor plugin.json, marketplace.json, harness.toml)
• [Unreleased] retains the R15 guardrail + glossary content for the next release.

Rebuild the bundled harness binaries before release

With this manifest bumped to 4.16.1, the Claude plugin archive still ships the tracked bin/harness* runtime; docs/distribution-scope.md marks those binaries as distribution-included and scripts/build-host-plugin-dist.sh copies them into the package. I checked the current tree and ./bin/harness version plus the platform binaries' embedded strings still report 4.15.0, so users installing this tag will see a 4.16.1 plugin while actually running the 4.15.0 hook/CLI and will not get the Phase 94 fixes unless the binaries are rebuilt and committed.
