Summary

Phase 94 open issue triage patch release.

Move 5 Fixed entries that were added to [4.16.0] after the v4.16.0 release into a new [4.16.1] - 2026-06-19 section, and bump version to 4.16.1.

Issues addressed

• harness-review and harness-release do not persist the review verdict to review-result.json, so an APPROVE produced outside harness-work cannot pass the commit guard #218 — harness-review / harness-release APPROVE was not persisted to .claude/state/review-result.json, causing commit guard to block subsequent git commit
• PostToolUse cleanup deletes the approval on every commit, so in harness-release's multi-commit flow (work + bump) the later commit is blocked by the commit guard #219 — release bump commit blocked by approval consumption (bookkeeping-only commits now exempt)
• Skill listing budget overflow: 28 skills sent to LLM = 9,089 chars (6,000 budget) #200 — Skill listing budget overflow short-term mitigation (top 10 verbose descriptions trimmed)
• English version #173 — Language / i18n setup documentation reachable from README (docs/i18n.md)
• Trigger ( This request triggered cyber-related safeguards ) #172 — Reviewer cyber-related safeguard mitigation (narrow security finding instruction + docs/known-limitations.md)

Version bump

• VERSION: 4.16.0 -> 4.16.1
• All version surfaces synced (plugin.json, codex/cursor plugin.json, marketplace.json, harness.toml)

Include adapter manifests in bookkeeping exemption

When the version bump includes this file (and the cursor/marketplace manifests updated in this same release), it no longer matches the bookkeeping-only exception: scripts/pretooluse-guard.sh only allows VERSION, .claude-plugin/plugin.json, harness.toml, and CHANGELOG.md, and PostToolUse cleanup uses the same list. In the harness-release multi-commit flow after the work commit has consumed the approval, this release bump is treated as a code-change and the git commit is denied or clears approval, so the #219 unblock still fails for releases that sync the adapter manifests. Please extend the exemption to every release version surface or avoid staging them in the automatic bump.
