Add Sara Stoudt's Shiny dashboard to the package

For the 2018 hackathon @sastoudt wrote a Shiny dashboard, see here: https://github.com/rlgbtq/TDoR2018/tree/master/shinyDashboard

We can add this to the package and make sure it works with the updated data.

@sastoudt we didn't get to this in the 2019 hackathon, but I found this useful guide on how it might be incorporated in the package if you or anyone else would like to submit a pull request: https://deanattali.com/2015/04/21/r-package-shiny-app/.

Note that Age_min and Age_max have been renamed Age min and Age max for consistency with other variable names.
