Show breadcrumbs on "Add Note" page

When adding/editing a note, there's no indication of how that note relates to anything else, except for the subset of users listed in the "Select User" box.

I think the information is there, it just needs to be displayed. Some ideas:

• Add name to first sub-heading, i.e., "Create Note in " or "Update Note in "
• Add a pre-filled entry to the form that is auto-filled with
• Add a redirect link <- Go back to Project details
