Salsa Sign Up Form

Hello,
I am trying to integrate my

1. The "Notes" section is very small, is there a way to make it a larger box?
2. When I use this sign up form in Salsa the button says "Submit" but here it says "Sign up" how do I have it say "Submit?" The words "Sign up" make it seems as though they are agreeing to sign up for something and it's possible they are just submitting a comment
3. Is it possible to change the notes field to say "Questions/Feedback?"
4. When I hit "Sign up" it just hangs and says "Saving... one wait one sec" but nothing else happens
