docs(revamp-G): operations index, conditions TrustGraph, CLI count - #201

Wave G of the v1.0 docs revamp - operations, API hygiene, CLI count correction. Changes: docs/operations/README.md (new) - the operations index page was missing entirely. Added a single-table landing page that routes to each of the nine existing operations pages and surfaces the day-to-day operator commands (status, logs, policy show, operator TUI). Also points at conditions.md as the canonical reference for every status condition the controller emits. docs/api/conditions.md - added the TrustGraph CRD section (GraphActive / GraphEmpty / Resolving / RollupComputing / PeerUnverified / RegistryUnreachable). The doc previously listed every CRD except TrustGraph, which was a bug. docs/cli-reference.md - corrected the command count in the opening paragraph from 23 to 26 (matches the actual command count in cli/src/commands/) and re-pointed the quick-start link from 'azureclaw up' to 'azureclaw dev' so the entry point matches the post-revamp Getting Started narrative. The remaining operations pages (a2a-gateway, branch-protection, byo-strict, chaos-tier, gitops, helm-packaging, image-versioning, secret-rotation, supply-chain) are accurate as-is and have not been touched.
