Ethnicity issue

For Hispanic, the code is only looking at the race_cd. This only give you ‘Yes’ or 'No-Information’ as values.

Additionally, I think it would make more sense to move the ethnicity information to an ethnicity_cd column in i2b2. Many of the GPC sites have implemented it this way.

At KU, we added ethnicity as a column to the i2b2 patient dimension. I modified the CDM ETL to pull ethnicity from the new column. njgraham/i2p-transform@71d515b.
