docs: update white paper with 3 new fix tasks and fresh IR metrics

- Task counts: 170 SDLC (was 167), ccb_fix 25 (was 22), total 251
- Fix suite results: n=25, baseline=0.479, MCP=0.491, delta=+0.012
- IR metrics: updated from n=41 to n=594 computable tasks with full
  pipeline output (file recall, MRR, MAP, nDCG, utilization probes,
  error taxonomy)
- Retrieval-outcome correlation: Spearman rho=0.078 (p=0.737)
- Development timeline extended through Feb 26
