align layouts of the Sign in, Create Account, Password Reset pages

Consistent alignment of the main elements on the Sign in, Create Account, Password Reset pages would make for a better UX experience until a fuller redesign can be done. Visually it's pretty clunky right now.

The fields, labels, buttons and links in the main section of Sign In page are all left-aligned and neatly stacked vertically, ie,:

Adjust the elements of the Create Account and Password Reset pages to follow this layout pattern.

Add more, even spacing between elements.

Also, the buttons should be Bootstrap-styled.
