This is a follow-up for #4790

Here are some ideas about improving protocol support logic in fine-grained mode:

Rename/split _cache_dir_prefix
Factor out logic in update_protocol_deps into flush_protocol_deps and add_protocol_deps
Move fine-grained protocol tests to a separate test file
Split TypeState into SubtypeCache and ProtocolDepsManager
