User Approval for Indexing Large File Additions in Watch Mode

When running in watch mode, if a large number of new files are created within a short time (e.g., due to file copies or package manager dependencies that are not in the currently ignored paths), the indexer should prompt the user for approval before including them.

This feature prevents unnecessary indexing of transient or irrelevant files, improving performance and keeping the index relevant.
