test(mcp): add comprehensive tests for structuredContent feature

- Test default behavior (useStructuredContent=false)
- Test enabled behavior with single/multiple content scenarios
- Test falsy value preservation (0, false, '', null)
- Test undefined structuredContent handling
- Test mixed scenarios and edge cases
- Test disabled flag with structured content present
- All 8 test cases pass with full coverage
