Add a "Copy as Markdown" option in the answer context dropdown menu

Currently when copying the answer text a lot is missing, it would be nice to be able to copy the markdown output to keep the original formatting of the answer text.

yeah this is a big pain point in UX right now that should be easy to address

to better illustrate - right now the only way to copy over a response from Codex is to select and copy text

you get something like:

Made createTerminationState idempotent for sentinel cleanup by guarding it per-path at module scope, so repeated calls won’t delete an active sentinel. This keeps the safety of a singleton without forcing one, and still allows explicit cleanup via ensureSentinelRemoved.

Details

termination.ts now tracks cleaned paths in a module-level Set and only calls removeSentinel once per path.
If you want a stricter singleton API (e.g., getTerminationState()), I can add that too. Suggested next steps:

If you want the singleton helper, tell me the preferred export location/shape.
If you want this mirrored in built lib/dist artifacts, I can update the build outputs.

unless you add the formatting back in yourself manually, and that's super annoying to have to do, while leaving it as-is often heavily impacts readability
