Follow-up to nextstrain/docs.nextstrain.org#223 which added docs for weighted sampling, but only with simple examples. The updated docs should describe how to create a weights file with the so-far-undocumented formats for month (YYYY-MM) and week (YYYY-WW) as well as multiple weighted columns.
