Create source/categories/index.md, and input the following into index.md:

---
title: All Categories
data: {{date}}
type: "categories"
---

Do the same for tags, then in _config.yml:

category_dir: categories
theme_config:
  ...
  menu:
    tags: /tags/ || fa fa-tags
    categories: /categories/ || fa fa-th

Should do the trick.
