On my map dashboard everything is ok.
Map is the most problematic part - because Elasticsearch must have mapping configured for index. If you stricly follow the instruction, send logs to new,clean elasticsearch instance then attached fields.yml is responsible for configure that and everything should be ok.
If there are already other filebeat-* indices (even removed ones) - then mapping from fields.yml won't be applied as it was done when first log was sent to it - and mapping is default from filebeat which is not ok for squid.
Other option is that remote IP addresses have no geo localization (usually happen for local IP addresses like 192.168.0.0/16 etc).
