docs(out-of-scope): name the two unratified entries in the ledger README

The README said every file is a settled rejection, while machine-profile.md
and shared-surface-instruction-governance.md are agent proposals awaiting an
owner ruling (#4666, #3568). Describe the directory as it stands. Keeping,
moving, or deleting either entry stays the owner's call.
