CFList type 0 is not properly processed for regions with 2 default frequencies

In looking at #739, it became clear that CFList processing for AS923 is broken; we're supposed to set "channels" 2..6, but we always set channels 3..7. Need to condition on number of default channels. Again, it's odd that this got through the compliance tests.
