Implement controller k8s deployment as Helm chart #48

Supports #37.

The Lyra controller should be deployable as a Kubernetes deployment. This means figuring out what deployments, statefulsets, pods, secrets etc are required to deploy the Lyra controller and how to package it together. The initial proposal here is to package it as a Helm chart.
