[11.x] Add support for specifying schema name on SQL Server framework#49965
[11.x] Remove Doctrine DBAL framework#48864 the alternative approach for inspecting databases was not clear for package maintainers (e.g. barryvdh/laravel-ide-helper#1510)

We may also add a section for these new schema methods to Inspecting Your Databases if you want?
