Handle missing pdbx_entity_nonpoly.ma_model_mode on read

ModelCIF defines pdbx_entity_nonpoly.ma_model_mode as mandatory, but when reading non-compliant files (or plain non-ModelCIF files when using make-mmcif.py) it may be missing.
