Request ID header only in response headers, also not work for forbidden requests

Hello,
I compiled this module in NginX and I would like to ask for more information how the module works exactly.

1. I only see the request_id header in the response headers, not in the request headers - why and is it normal?
2. If I open a page that is with status 403 the header is missing, but I need the module to work exactly for this pages. Can I use this module for this purpose?
3. The request_id does not appears in the error_log and mod security logs of nginx, although everything from the documentation is configured properly. Shouldn't it appears in all logs?
