Change password modal doesn't auto close

Change password modal for organization does not automatically close on incorrect password. When a user enters the current password and a new password and attempts to submit the change, the 'confirm change' password modal needs to automatically close whether the change occurs or not.

The failure needs to be properly visible to the user, and give the current password input box focus to enable the user to quickly fix the mistake.
