Acceptance Criteria

When clicking the save button:

  1. the form data is saved to the participant table in the database
  2. the user is notified that the record was saved
  3. the user is redirected to the new participant profile pre-loaded with the just-saved data

Development Notes

  • Create a client side contract that shows the types of the intake form?
