Add upgrade guide regarding azure container name change #6118

What this PR does / why we need it:

In #6015 we changed the default value of the Azure container name from cortex to loki. If anyone was relying on this value, they will need to manually specify it when upgrading to the latest version.

Approving this PR and hoping that you'll put in my suggested change.


#### Default value for `azure.container-name` changed

This value now defaults to `loki`, it was previously set to `cortex`. If you are relying on this container name for your chunks or ruler storage, you will have to manually specify `-azure.container-name=cortex` or `-ruler.storage.azure.container-name=cortex` respectively.
