Originally I was planning per-page, but in practice this proved overkill so I went per-site, i.e., {scheme}://{hostname}.

Currently calling the feature "per-page" will breed confusion.

Need to revise both code comments/UI/wiki to ensure "per-page" expression is no longer used.
