Is there a specification for how URL slugs for headers are generated? #118

I've encountered issues with https://github.com/jonschlinkert/markdown-toc not generating the correct URL slug for headers (for instance, it currently fails to remove HTML entities like > and gt winds up in the slug it generates). It would nice if this could be formally specified so we can guarantee the correctness of libs like markdown-toc.
