major re-writes

Re-wrote major parts of the RackServer class, moving much of the logic to middleware.

First working release

This is the first working release for Anorexic.

All the bugs I found in my testings were squashed...

On my machine, Anorexic v. 0.2.1 out-performed Rails 4.1.4 by a... lot. It looked almost 4 times faster (3.87 times faster)... which must be wrong - but since both my server and my machine are macs (my server is a mac mini), it's great for me.

I marked this release as pre-release mainly because I know I'm not good at testing (I don't even know how to write automated tests, so I just do it the long way, spending tons of hours on checking things and coding tight).

please report any bugs and push and changes to the code.
