Refactor Publish-Module into smaller responsibilities

This code base has gotten long in the tooth and it desperately needs better test coverage and more narrowly defined test cases.

Publish-Module is a good place to start.

One example refactoring: Extract nuspec logic into New-NuspecFile and ConvertTo-NugetPackage
