FEAT-132 Add a "Open in PDF" link to the recent book references

Information
As of now, the site adds "Read x in y" references to complete the book contents about lore and such, and it can be cumbersome to open each book or PDF file on the proper page each time.
A link could be added in these references to open the PDF file in the correct page automatically (sorry physical book owners)

Who would use it?
People who self host the site, as it would be needed to copy their Paizo PDFs into their server.

How would it work?
Check if the site is being self hosted. If so, add a link to open the correct PDF in the correct page to the reference.
Bonus points if they are only added if the PDF files exist in the server in order to not have dead links. People might only own some of the PDFs.

Why should this be added?
Less fiddling with files, more s p e e d.
