I may be wrong - but I see strong indications that I even have to close the entire XrmToolBox for the list to refresh, which would mean they are cached somewhere globally and not in the currently executing instance of the tool.
The Refresh button does not refresh available messages, so if you add a new Custom Action you need to close and reopen the tool to see it.
Add an option to refresh messages, or include it in the existing refresh process.
