Pull request review workflow

We use the gh-pr-query tool to facilitate pull request reviews. See the README file for an overview and installation instructions. See the associated wiki page for details on our workflow for pull request reviewers and authors.

One item of particular note: In general, the reviewer who made a comment should be the one to check off the relevant checkbox; the pull request author should NOT check off checkboxes themselves.
