org.csstudio.diag.epics.pvtree issue if simplepv uses pvmanager

org.csstudio.diag.epics.pvtree is currently broken when configuring org.csstudio.simplepv/default_pvfactory=pvmanager.

Works fine with org.csstudio.simplepv/default_pvfactory=utility_pv.

Alternatively, org.csstudio.diag.epics.pvmanager.pvtree works, but will issue many warnings "org.epics.pvmanager.PVReaderDirector (isActive) - PVReader wasn't properly closed and it was garbage collected."

This bug has been fixed. I will push it in until the release is done.

The PVManager warnings "PVReader wasn't properly closed.. " are gone with the PVManager update from #144, so this was indeed a bookkeeping issue in PVManager, not PVTree.
